Appendix F – Minor Child Guest Parental Agreement
Please read carefully before signing.
This document includes a release of liability and waiver of certain legal rights. It also includes indemnification and hold harmless provision.
This agreement applies to minor children participating in sporting, recreational or educational activities at Anthem Ranch including, but not limited to, the use of the swimming pools and other activities.
WARNING
There are dangers inherent in usingthe facilities atAnthem Ranch. There are serious dangers inherent in the use of swimming pools. These include death or injury that result from:
- Drowning or near drowning,
- Tripping or falling,
- Cardiovascular or other health complications,
- Interaction or collision with other users,
- Malfunction of the facility equipment,
- Reckless or negligent conduct of other users.
The above is not a complete list and only provides examples of some of the dangers and injuries that users of our swimming pools and other facilities should be aware of.
I UNDERSTAND
1. Guests must abide by the rules posted at the facility. Failure to do so may result in denial of the guest’s privileges or removal of the Guest from Anthem Ranch.
2. The Center for Disease Control has determined that drowning is the #2 cause of death in children under the age of 15. Thus, an adult must be responsible for and always supervise minor children when they are using the pool or other facilities.
3. When children are using the pool facilities, the supervising adult SHALL ALWAYS:
a. Be in the pool within arm’s reach of their children who are not proficient swimmers, and
b. Observe and supervise the minor children
AGREEMENT
In consideration for myself and myminor children(named below) being permitted to use the facilities at Anthem Ranch, I RELEASE and WAIVE any claim of liability that I, or any minor
child of mine, may have against Anthem Ranch Community Association, CCMC, or the
directors, officers, members, employees and agents of both organizations (collectively, the Protected Parties) with respect to any injury, illness, damage or death occurring to any of my minor children as a result of participating in sporting, recreational or educational activities including, but not limited to, the use of the swimming pools and other facilities, at Anthem
Ranch.
Furthermore, on behalf of myself and my minor children, I agree to INDEMNIFY, DEFEND ,and HOLD HARMLESS the Protected Parties with respect to any injury, illness, damage or death occurring to any of my minor children as a result of participating in sporting, recreational or educational activities including, but not limited to, the use of the swimming pools and other facilities Anthem Ranch.
